Appointments:

Because your appointment time is reserved specifically for you, we respectfully request that you give us favor of two business days notice if you must cancel or reschedule an appointment.   Although we do provide a courtesy reminder call, text, or e-mail prior your appointment, please consider your appointment confirmed at the time it is made. 

We understand that emergencies occasionally arise; in this case please notify us as soon as possible so that we may offer the time that was reserved for you to another patient.

 

 

 

 

 

Your Dental Insurance:

For your convenience and benefit, we are "In Network" or  " a participating dental provider" with CIGNA, DELTA DENTAL (as a Premier Provider), PREMERA, and REGENCE. 


If you have other dental insurance, we are happy to submit your claim forms for you, and to communicate with your insurance company to help you obtain all of the insurance benefit to which you are entitled. 

We will always provide you with a cost estimate in advance of your treatment if you request, and we will try to determine how much dental insurance benefit will be estimated to you. However, not all non-participating insurance companies make that advance information available to us. 

IMPORTANT: If you would like us to confirm your insurance eligibility and benefit level prior to your first visit, please provide us with your insurance information several days in advance of your first visit.

For your non-insured portion, we accept most major credit cards, personal check or cash; at the time of service.
